MM Contact Form

MM Contact Form can manage multiple contact forms, plus you can customize the form and the mail contents flexibly with simple markup. The form supports Ajax-powered submitting, CAPTCHA, Akismet spam filtering and so on.

Customizing a Form
You may feel that the default form is too simple for you and you want to add more fields to it. You can edit the form template in the admin screen and add other fields.
To add fields to a form, make tags for them and insert them into the ‘Form’ field. You’ll find unfamiliar codes in the ‘Form’ field, for example, [text* your-name]. These codes are called “tags” in the vocabulary for MM Contact Form.

Customizing Mail
You can edit mail templates in the ‘Mail’ field set as you did with the form template. You can use tags there as well, but note that tags for mail are different from those tags for forms.

Setting Default Values to the Logged-In User
You may need to create a contact form targeted at users with accounts on your WordPress site. You already have basic information about the user (name, e-mail, etc.) so asking users for this would be redundant.
